Questionnaires
Young People (Shorter Version)
There are two versions of the young people's questionnaire, each
young person that wishes to take part only needs to complete one
questionnaire either the standard questionnaire or the shorter
version. QNIC would advise that children's units use the shorter
version which contains less questions and also uses simpler
language.
The findings from this questionnaire will be looked at on the
unit's peer review day during the session dedicated to young people
- if you would like to take part in the session for young people on
the peer review day please talk to your named nurse/keyworker.
Please note: no staff will be present during that session so you
can be sure that your answers will be kept confidential.
Guidance for inpatient staff: Response rates
should be 50% of the total number of beds on a
unit, e.g. If you have 12 beds then at least 6 questionnaires
should be completed.

The findings from this questionnaire will be validated on the
unit's peer review day - if you would like to take part in the
session for parents and carers on the peer review day please speak
to a member of the staffing team at the inpatient unit.
Guidance for inpatient staff: For accreditation
the QNIC team will need to receive questionnaires from at
least 5 sets of parents/carers.

Guidance for inpatient
staff: All current clinical staff at the
unit (excluding bank and agency staff) should individually complete
this questionnaire. The findings from this questionnaire will be
validated on the review day during the frontline staff meeting.
Guidance for inpatient
staff: Ward managers should complete this questionnaire
in addition to the generic staff questionnaire.
The findings from this questionnaire will be validated on the
review day during the ward managers meeting.
Guidance for inpatient
staff: Teachers only need to complete
this questionnaire they do not have to complete the generic staff
questionnaire. The findings from this questionnaire will be
validated on the review day during the frontline staff meeting - if
possible a representative from the teaching staff should be present
during this session.

staff: For accreditation at least one
referrers questionnaire will need to be completed.
Guidance for inpatient
staff: For accreditation at least one
commissioners questionnaire will need to be completed.
Additional Questionnaires
In addition to the individual
questionnaires above the following questionnaires need to be
completed:
Guidance for inpatient
staff: This questionnaire should be completed for 5 sets
of case notes. So the questionnaire should be completed 5
times, one per set of notes. The selection of the 5 notes
should be taken at random, but discount the most recent cases
because they may not contain all the information required. If you
have difficulties identifying which notes to select or you
cannot select 5 sets of notes please get in contact with the QNIC
team on 020 7977 6693.
It is recommended that 1-2 people
from the team that have a good working knowledge of the format of
patient records complete this questionnaire. It would also be good
if these individuals are available on the peer review day to aid in
validating this questionnaire.
Guidance for inpatient
staff: This questionnaire should only be submitted
once per inpatient unit.
It is recommended that 1-2 people from the senior staff team
complete this questionnaire, and if possible are available on the
review day to aid in validating it.
Guidance for inpatient staff: This is a word
document that needs completing and emailing to the
QNIC team at QNICaccreditation@cru.rcpsych.ac.uk.
One staffing numbers sheet should be submitted per
inpatient unit.
Guidance for inpatient staff: Some supporting
documentation will need to be seen by the QNIC review team during
your accreditation peer review visit to evidence that you meet
particular criteria. This document provides you with a list of the
criteria that will be checked and which will need supporting
documentation to be available on the review day for the team to
view.
