Why have I been asked to fill in this
questionnaire?
Your CAMHS team has decided to apply for an
accreditation award from the Quality Network for Community CAMHS
(QNCC). To decide whether the team should be accredited, QNCC need
to find out about the views and experiences of all the people
concerned, including young people, parents and carers, staff,
commissioners and partner agencies.
Please click on the link below that relates to
you, and you will be asked for your password. Your answers will
help the CAMHS team to improve their service. Your answers will be
completely confidential and will not affect the care you receive or
work you do with CAMHS. The questionnaire will take about 5 to 10
minutes to fill in.

In addition, the manager(s) of the CAMHS team
should ensure the following are completed:
A nominated member (or members) of staff
should complete the checklist below, which covers policies,
training and inter-agency agreements held by your service. It might
be useful to have policies and training records to hand. The
information given in this checklist will be validated on the day of

about 20 minutes to fill in:
Nominated members of staff should complete the
audit tool below for 10 separate sets of case notes. It will take
about 15 minutes to complete for each set of case notes.
