Members of ECTAS can securely submit audit data and questionnaire responses on the links indicated here.

There are seven sets of self review tools collected:

• One Environment & Facilities Audit; 

• One Staff Questionnaire;

• One Documentation Audit;

• Five Observation Audits;

• Between 10-20 Health Record Audits;

• Five Referring Psychiatrist Questionnaires;

Please ensure that you use the correct log-in details.  You should have received these on the cover letter with your self review pack, and by email. If you do not know them, please contact the ECTAS team.
