Data collection

Welcome Teams!

Here you can access the self review data tools for completion online.  You will need to use your username and password we provided in order to access them.  If you do not know your password, please see your project lead or contact the PLAN team.  The questionnaires are returned directly to the PLAN central team and are not seen by any of the liaison team leads.

• Acute colleague questionnaire (acute staff who refer to the liaison team)
• Team checklist
• Case note audit
• Patient and carer questionnaires can be found at http://www.plan-info.org/

Please note that 100% of liaison staff should complete the questionnaire, and you should aim to have at least two-thirds of your acute colleagues complete their questionnaire.
