Members of the Memory Services National Accreditation Programme can securely submit audit data and questionnaire responses here.

 

 

There are three sets of login details:

 

  • one set for the Case Note Audit and Checklist;

 

  • one set for the Staff Questionnaire;

 

  • one set for the Referrer Questionnaire.

 

 

Please ensure that you use the correct log-in details.  You should have received these on the cover letter with your self review pack, and by email. If you do not know them, please contact the MSNAP team.

NB: If you wish to switch between questionnaires (e.g. between case note audit and staff questionnaire), please ensure you log out before you attempt to log in and access a different questionnaire.  Remember that there are different login details for the staff questionnaire and case note audit/checklist.
