Perinatal Accreditation Tools

 

This page is dedicated to the inpatient units taking part in the Perinatal Quality Network accreditation visits during Cycle 5 of reviews. The most important part of the accreditation process is to ensure that the appropriate data is collected before and during the review day.

 

Why have I been directed to this webpage?

 

The inpatient unit that you are currently staying in, know someone staying in, or working with is undergoing an accreditation assessment. This means an independent organisation- the Quality Network for Perinatal Mental Health Services at the Royal College of Psychiatrists- is assessing the performance and quality of the service provided. A vital part of this process is to obtain feedback from as many people who are currently have a link to the unit.

 

What this page does and how it works   

This page provides links to a range of online questionnaires designed in order to obtain an understanding of how the service is performing.

  • To start, scroll down the screen and locate the appropriate questionnaire by clicking on the title that relates to you, e.g. for the Patients questionnaire click on the heading Patients.
  • To submit data, click on the appropriate questionnaire and go through the answers one by one (please note: all questions must be answered before you can move onto the next one, except for the questions in the patients and family/partners questionnaires). Once you have answered the questions please press 'Submit' at the bottom of the questionnaire.
  • After you have submitted your answers, the questionnaire will automatically be sent to the Perinatal Quality Network team to collate and analyse.
  • Please note that all questionnaires are kept confidential and that we do not collect any identifiable data.

 

Further information for the inpatient staff team

  • There are some additional questionaires towards the bottom of the page that need to be completed for each inpatient unit- the staff team should decide who is the most appropriate person to complete these.
  • There is guidance under each questionnaire for inpatient staff about the response rate required - the Perinatal Quality Network team will let the inpatient staff know during the data collection period if there are any particular questionnaires that are not being returned.

 

 

Questionnaires

Patients

The findings from all the patient's questionnaires will be considered at on the review day during the session dedicated to patients. If you would like to take part in the session for patients on the review day please talk to your key worker/named nurse. Please note: no staff will be present during that session so you can be sure your answers will be kept confidential.

 

Guidance for inpatient staff: Response rates should be 50% of the total number of beds on the unit, e.g. if you have 8 beds then at least 4 questionnaires should be completed. If patients would prefer to fill out paper copies of this questionnaire please contact the Perinatal Quality Network team on 0207 9776680 or email ecollins@cru.rcpsych.ac.uk

 

Family/Partners

The findings from this questionnaire will be validated on the unit's review day -  if you would like to take part in the session for family members or partners on the review day please speak to a member of the staffing team at the inpatient unit.

 

Guidance for inpatient staff: For accreditation the project team will need to recieve questionnaires from family/partners representing at least 50% of patients. If you need paper copies of this questionnaire to hand out to parents/carers please contact the project team on 0207 9776680 or email ecollins@cru.rcpsych.ac.uk

 

Staff

Guidance for inpatient staff: ALL current staff at the unit (excluding bank and agency staff) should individually complete this questionnaire. The findings from this questionnaire will be validated on the review day during the frontline staff meeting.

 

Ward Manager

Guidance for inpatient staff: Ward managers should complete this questionnaire in addition to the generic staff questionnaire. The findings from this questionnaire will be validated on the review day during the ward manager's meeting.

 

Referrers  

Guidance for inpatient staff: For accreditation at least two referrers questionnaires will need to be completed.

In addition to the individual questionnaires above the following questionnaires need to be completed:

 

Case Note Audit

Guidance for inpatient staff: This questionnaire should be completed for 5 sets of case notes so the questionnaire should be completed 5 times, one per set of notes. The selection of the 5 sets of notes should be taken at random, but discount the most recent cases because they may not contain all the information required. If you have any difficulties identifying which notes to select or you cannot select 5 sets of notes please get in contact with the project team on 0207 9776680.

 

It is recommended that 1-2 people from the team that have a good working knowledge of the format of patient records complete this questionnaire. It would be useful if these individual are available on the peer review day to aid in validating this questionnaire.

 

Policy Audit

Guidance for inpatient staff: This questionnaire should only be submitted once per inpatient unit.

 

It is recommended that 1-2 people from the senior staff team complete this questionnaire, and if possible are available on the review day to aid in validating it.

 

Staffing Numbers

Guidance for inpatient staff: This is a word document that needs completing and emailing to the project team at ecollins@cru.rcpsych.ac.uk. One staffing numbers sheet should be submitted per inpatient unit.

 

Supporting Documents

Guidance for inpatient staff: This link directs you to a letter which details all the documentation that should be emailed to the project team, ecollins@cru.rcpsych.ac.uk prior to the review day, as well as which documents shoud be made available for the review team during the review day.
