The findings from all the patient's questionnaires will be
considered at on the review day during the session dedicated to
patients. If you would like to take part in the session
for patients on the review day please talk to your key
worker/named nurse. Please note: no staff will be present during
that session so you can be sure your answers will be kept
confidential.
Guidance for inpatient staff: Response rates should be 50% of
the total number of beds on the unit, e.g. if you have 8 beds then
at least 4 questionnaires should be completed. If patients
would prefer to fill out paper copies of this questionnaire please

Guidance for inpatient staff: For accreditation
the project team will need to recieve questionnaires from
family/partners representing at least 50% of
patients. If you need paper copies of this questionnaire

Guidance for inpatient staff: ALL current staff
at the unit (excluding bank and agency staff) should individually
complete this questionnaire. The findings from this questionnaire
will be validated on the review day during the frontline staff
meeting.

the generic staff questionnaire. The findings from this
questionnaire will be validated on the review day during the ward
manager's meeting.

In addition to the individual questionnaires above the following

Guidance for inpatient staff: This
questionnaire should be completed for 5 sets of case notes so the
questionnaire should be completed 5 times, one per
set of notes. The selection of the 5 sets of notes should be taken
at random, but discount the most recent cases because they may not
contain all the information required. If you have any difficulties
identifying which notes to select or you cannot select 5 sets of
notes please get in contact with the project team on 0207
9776680.
It is recommended that 1-2 people from the team that have a good
working knowledge of the format of patient records complete this
questionnaire. It would be useful if these individual are available
on the peer review day to aid in validating this questionnaire.
Guidance for inpatient staff: This

inpatient unit.
