About the Audit

The Royal College of Psychiatrists' Research and Training Unit established the National Audit of Violence in 1999. 265 inpatient units participated in the 2003/5 wave that ended in March 2005.

Its success led the Healthcare Commission to fund a new wave of work, which began in Spring 2006 and finished in October 2007. The Healthcare Commission funding enabled all NHS trusts and independent sector providers of in-patient mental health services in England and Wales to select three wards to take part.

Who took part?

Of the three wards we asked each participating trust/organisation to enrol, we requested:

  • One acute psychiatric ward;
  • One ward for older people with mental health problems;
  • One 'other' ward of their own choice i.e. this could be another ward of the types above or another adult specialty.

A total of 67 trusts/organisations took part in the audit, enrolling a total of 211 wards.

What did it involve?

The audit methods were refined and proved popular and feasible for participating services. The standards used in the previous phase of the audit were updated to incorporate the more recently published NICE Guideline 'Violence - The short-term management of disturbed/violent behaviour in inpatient psychiatric settings and emergency departments', (NICE 2005).

Data Collection included

  • Contextual information to support benchmarking and networking;
  • Module 1: survey of the experiences of staff, patients and visitors;
  • Module 2: audit of the environment
  • Module 3a: review of a series of violent incidents
  • Module 3b: case note/drug chart audit of the use of rapid tranquillisation
