• Retirement declaration form  
  When you retire from Professional Practice you may be entitled to pay a lower subscription fee. This "retired fee" applies when expected income from any continuing professional sources is not expected to exceed £9,000 in the year from 1 January 2012.  This earning figure refers specifically to ongoing gross income from professional practice- pension income is excluded from this calculation.  To apply please complete and sign this form and return it to the Membership Data Office at the College.

• Reduced rate form 
  UK Fellows, Members, Specialist Associates and Affiliates can apply for a reduction of 50% in their fees if their earnings are below £34,061 in the subscription year 2012.  To apply please complete and sign this form and return it to the Membership Data Office at the College.  Please note, this form needs to be completed for every year that you wish to be paying a reduced rate of subscription.

• Paying membership subscription by direct debit
  Paying by Direct Debit can save UK members up to £25 on their annual subscription.

• Advance travel & subsistence approval & booking form  
  Please complete this form where you may be travelling on behalf of the College. The College will then make the booking for you.
