Intake
How to Complete your CT1 Application Online
The form is divided into sections with an
explanation at the top of each page. Guidance and help text is
provided throughout the application wherever you see the
information symbol.
You can navigate to each section using the
links in the menu on the right. When you have completed a section,
it will be marked with a green tick . Incomplete sections will be
marked with three orange dots and the pages you have not yet
started completing will have a red cross. You will not be able to
submit your application until every section has a green tick,
although you will be able to edit sections marked with green ticks
before submission.
If a question requires a narrative answer, you
may wish to prepare your answers in advance and then paste the text
into the relevant text box when the application window opens.
IMPORTANT
NOTE: To avoid corrupting your text during the
‘copy-and-paste’ process please paste your text into a Notepad
document first (this will strip the text of all the hidden
characters) and then copy it from Notepad into the text box in your
application.
PERSONAL INFORMATION SECTION
In this section you will be asked to provide
the following information:
- Your date of birth
- Your current residential
address (and correspondence address if different)
- Your home and mobile telephone
numbers
If you wish to train on a less than full time
basis or would like to defer your start date you will need to
provide relevant details in this section. Information regarding
adjustments required due to disability will also need to be
provided here.
MEDICO-LEGAL DETAILS
SECTION
In this section you will be asked to provide
the following information:
- Your English language skills
and evidence you will submit to support it
- Your nationality
- Your UK national insurance
number (if applicable)
- Your country of
residence
- Your current immigration
status if not a EU/EEA resident
You will also be asked to provide information
regarding transportation during your training. As a trainee doctor,
you will be expected to be able to attend to emergencies and
provide domiciliary care which may include out of hours visits if
you are based in a community post rather than a hospital. If you
are not a car-driver, you are expected to provide a car and driver
at your own expense in order to meet this requirement.
FITNESS TO PRACTISE SECTION
This section used yes/no dropdown menu
answers. You will be asked a series of questions about your
criminal convictions, professional conduct, driving offences, etc.
You MUST provide details of all criminal convictions including road
traffic offences. You do NOT need to declare parking offences, for
example, but you must declare any offences of driving with excess
speed. Please see the Applicant Guide
for further details.
CLINICAL REFERENCES SECTION
This section asks you to list three referees.
One MUST be your current or most recent supervisor, familiar with
your clinical development. The other two referees should relate to
your most recent previous posts. For all three referees you will
need to provide their address, email, contact number and dates you
worked with the referee. Please see the Applicant Guide
for further details.
FOUNDATION COMPETENCES
SECTION
In this section you will need to provide
details of the evidence you have which demonstrates you have
achieved your foundation (or equivalent) competences. Detailed
information about foundation competencies can be found on the
Foundation
Programme website. The way in which you can demonstrate your
foundation competences will depend on your individual
circumstances. Please see section 3.1.1 of the Applicant Guide
for full details of how to evidence your competences.
QUALIFICATIONS SECTION
In this section you will be asked about your
primary medical qualification as well as any other undergraduate or
postgraduate degrees and qualifications you might hold.
- Primary Medical
Qualification
This section uses dropdown menu options for
you to choose from. You will need to provide the following
details:
- Name of
qualification
- Start and
end date of the course of studies leading to the award of the
qualification (day, month, year)
- The
country where awarded
- Medical
school
- Date
qualification obtained (day, month, year)
- Undergraduate Degrees and
Qualifications
This section asks you to provide details of
your medical degree, any other undergraduate and/or postgraduate
degrees and qualifications you may have. Your primary medical
qualification is not scored, but additional qualifications may
receive a score.
You will be asked to select the correct
statement from a dropdown menu. You must then complete the text box
(word limit = 30) to give further information (e.g. title of course
/ degree, dates etc.). If you have further qualifications to add,
select ‘add qualification’. If you want to change it, select
‘delete qualification’ and re-enter, as appropriate.

EXPERIENCE SECTION
In this section you are asked to list all your
previous posts in reverse order, starting with the current / most
recent. You will need to document the name of the person you worked
for, the post you were in, the level you worked at and the name of
the Trust, together with dates. You must list each separate
component of your foundation posts, rather than ‘Foundation Year 1
post’ and ‘Foundation year 2 post’, each of which covers several
separate attachments. Select ‘Add new post’ for each post you wish
to list. If you make an error, select ‘Delete post’, and then
re-enter correctly.
The form will ask you if you have any gaps in
your career history. Please note that if you select ‘No’ and there
are any significant time gaps between the posts you have entered,
the system will not allow you to submit the form unless you have
provided an adequate explanation.
SUPPORTING INFORMATION
SECTION
This section has a number of important
questions which are scored at the shortlisting stage. Completion of
the text boxes is not mandatory if you have no evidence for the
section. Any claims you make in your application may need to be
supported by evidence at interview.
- Achievements outside Medicine (word
limit = 100)
This is a free text box. If you give one or
more examples of an achievement, ensure that you include details of
your role, the effort required, and the relevance of the
achievement in relation to psychiatry training. For example, just
stating that you were head boy / girl will not attract as many
marks as a statement that includes the relevance of this to your
application and future training.

In this section you will use the ‘drop-down’ list to select a
statement most relevant to your circumstances and provide details
in a free text box. Do not include audit presentations here, please

journals. If you are unsure, visit the journal’s website to
determine whether submissions are peer-reviewed.
Please provide full citation details (as in
PubMed, but excluding your own name) of any published work, specify
any abstracts and give a statement about your personal contribution
to the work in the text box.

short training-to-teach sessions (most Foundation programmes and
many undergraduate programmes include some mandatory training on
how to teach) do NOT count as formal training in this context. We
are looking for evidence of a more substantial training course in
teaching methods; typically, but not always, training in different
teaching methods lasting at least a week.

clinical work, relevant to a career in psychiatry.

In this section please describe how you
believe you meet the person specification requirements for the
psychiatry training programme. Include the particular skills and
personal attributes that make you suitable for a career in this
specialty. Please provide fresh supporting information. Do not
simply repeat the information you have already mentioned elsewhere
in the application form. Consider any activities you have
undertaken and achievements accomplished which demonstrate your
commitment to a career in psychiatry.
EQUALITY AND DIVERSITY MONITORING
SECTION
This section deals with equal opportunities
monitoring information required by the National Health Service
(NHS) to monitor their recruitment practices. In order to comply
with the obligations placed on them under equality legislation,
