NAPT data collection tools
Please note:
Three tools were developed to collect
data for NAPT from participating services, these include a
retrospective audit form, a service user questionnaire and a
therapist questionnaire. The tools included all the items necessary
to measure adherence to the NAPT audit standards. All NAPT tools
can be viewed and downloaded by following the links below.

this Tool or Tools that you are entering into a licence agreement

All information, software,
products and related graphics contained in the Audit Tool are
provided "as is" without warranty, including but not limited to the
implied warranties of satisfactory quality, fitness for a
particular purpose, title and non-infringement of third party
intellectual property rights.
In no event shall HQIP be liable
for any direct, indirect, incidental, special or consequential
damages for loss of profits, revenue, data or use incurred by you
or any third party, whether in action in contract, tort, or
otherwise, arising from your access to, or use of, the Audit Tool.
HQIP make no representations about the suitability, reliability, or
timeliness, and accuracy of the information, software, products and
related graphics contained in the Audit Tool. HQIP reserves the
right to make improvements, changes or updates to the Audit Tool at
any time without notice.
HQIP monitors and controls the licence
and use of each NAPT tool. For this reason, before the tool(s) are
downloaded we require an email is sent to the NAPT team on napt@cru.rcpsych.ac.uk. In your
email please include the title 'Tool licence agreement' in
the subject heading and in the contents of the email include the
following details:
- Name of your organisation
- Tool/tools to be downloaded
- Date of download

Therapist
questionnaire:
Please note that the
deadline for the therapist questionnaire was 30 September 2012. We
cannot accept any further submissions for this.
Service user questionnaire:
Please note that the
deadline for the service user questionnaire was 31 January 2013. We
cannot accept any further submissions for this.

Retrospective case record
audit:
Please note that the
deadline for the retrospective case record audit was 18
