QNIC Accreditation Tools


This page is dedicated to the inpatient units taking part in the QNIC accreditation visits during Cycle 12 of reviews. The most important part of the accreditation process is to ensure that the appropriate data is collected before and during the review day.

 

Why have I been directed to this webpage?

The inpatient unit that you are currently staying in or working with is undergoing an accreditation assessment. Which means that an independent organisation - the Quality Network for Inpatient CAMHS (QNIC) at the Royal College of Psychiatrists - is assessing the performance and quality of the service provided. Part of this process is to obtain feedback from as many people as possible who currently have a link to the unit.

 

What this page does and how it works:

  • It provides links to online questionnaires to obtain an understanding of how the service is preforming.
  • A dedicated questionnaire  is to be completed for all individuals involved in inpatient CAMHS - to access the appropriate questionnaire click on the title relating to you, e.g. for the young person's questionnaire click on the heading Young People.
  • There are also some additional questionnaires towards the bottom of this page that need to be completed for each inpatient unit - the staff team should decide who is the most appropriate person to complete these.
  • To submit data, click on the appropriate questionnaire and go through the answers one by one (please note: all questions must be answered before you can move onto the next one, except for the questions in the young people's and parents/carers questionnaires). Once you have answered the questions please press 'Submit' at the bottom of the questionnaire.
  • After you have submitted your answers to the questionnaire it automatically sends them to the QNIC team to collate and analyse.
  • Please note that all questionnaires are kept confidential and we do not collect any identifyable data.
  • There is guidance under each questionnaire for inpatient staff about the response rate required - The QNIC team will let the inpatient staff know during the data collection period if there are any particularly questionnaires that are not being returned.

Questionnaires

Young People

Young People (Shorter Version)

There are two versions of the young people's questionnaire, each young person that wishes to take part only needs to complete one questionnaire either the standard questionnaire or the shorter version. QNIC would advise that children's units use the shorter version which contains less questions and also uses simpler language.

The findings from this questionnaire will be looked at on the unit's peer review day during the session dedicated to young people - if you would like to take part in the session for young people on the peer review day please talk to your named nurse/keyworker. Please note: no staff will be present during that session so you can be sure that your answers will be kept confidential.

Guidance for inpatient staff: Response rates should be 50% of the total number of beds on a unit, e.g. If you have 12 beds then at least 6 questionnaires should be completed.

Parents/Carers

The findings from this questionnaire will be validated on the unit's peer review day - if you would like to take part in the session for parents and carers on the peer review day please speak to a member of the staffing team at the inpatient unit.

Guidance for inpatient staff: For accreditation the QNIC team will need to receive questionnaires from at least 5 sets of parents/carers.

Staff

Guidance for inpatient staff: All current clinical staff at the unit (excluding bank and agency staff) should individually complete this questionnaire. The findings from this questionnaire will be validated on the review day during the frontline staff meeting.


Ward Manager

Guidance for inpatient staff: Ward managers should complete this questionnaire in addition to the generic staff questionnaire. The findings from this questionnaire will be validated on the review day during the ward managers meeting.


Teachers

Guidance for inpatient staff: Teachers only need to complete this questionnaire they do not have to complete the generic staff questionnaire. The findings from this questionnaire will be validated on the review day during the frontline staff meeting - if possible a representative from the teaching staff should be present during this session.


Referrers

Guidance for inpatient staff: For accreditation at least one referrers questionnaire will need to be completed.


Commissioners

Guidance for inpatient staff: For accreditation at least one commissioners questionnaire will need to be completed.


Additional Questionnaires

In addition to the individual questionnaires above the following questionnaires need to be completed:


Case Note Audit

Guidance for inpatient staff: This questionnaire should be completed for 5 sets of case notes. So the questionnaire should be completed 5 times, one per set of notes. The selection of the 5 notes should be taken at random, but discount the most recent cases because they may not contain all the information required. If you have difficulties identifying which notes to select or you cannot select 5 sets of notes please get in contact with the QNIC team on 020 3701 2663.

It is recommended that 1-2 people from the team that have a good working knowledge of the format of patient records complete this questionnaire. It would also be good if these individuals are available on the peer review day to aid in validating this questionnaire.


Policy Audit

Guidance for inpatient staff: This questionnaire should only be submitted once per inpatient unit.

It is recommended that 1-2 people from the senior staff team complete this questionnaire, and if possible are available on the review day to aid in validating it.


Staffing Numbers

Guidance for inpatient staff: This is a word document that needs completing and emailing to the QNIC team at QNICaccreditation@cru.rcpsych.ac.uk. One staffing numbers sheet should be submitted per inpatient unit.


Supporting Documents

Guidance for inpatient staff: Some supporting documentation will need to be seen by the QNIC review team during your accreditation peer review visit to evidence that you meet particular criteria. This document provides you with a list of the criteria that will be checked and which will need supporting documentation to be available on the review day for the team to view.
