If you would like to join the Quality Mark scheme and take part in the forthcoming data collection period, please note that a minimum of two wards per Trust MUST sign up together to enable completion of the observational module making up part of the dataset required.  The Ward Manager and a Lead Consultant must also both agree to participate. Please consult or forward this Information Sheet for Ward Leaders to help you/them decide whether your ward'(s) participation is feasible. If everyone agrees, please complete this Joining Form and return to the Quality Mark Team, either by post or by scanning and emailing the completed forms. Please note that we will not be able to acccept forms that lack either the Ward Manager or Lead Consultant's signature.
