Dementia 100 Pathway Assessment Tool
Hosted by the NCCMH, the Dementia 100 is a self-assessment tool, developed by the NHS Midlands and Lancashire Commissioning Support Unit in partnership with the Department of Health and Social Care, that offers a set of 100 criteria against which Places can measure their current performance.
The Dementia 100 brings together multiple resources into a single, consolidated tool. Designed to reflect best practice and assess the effectiveness of dementia services, these criteria include 14 top-priority areas and support high-quality, person-centred and integrated care.
By helping Places and systems identify where improvement needs to be targeted, the tool continues the work of the Dementia Care Pathway, covering all elements of the Well Pathway from Prevention through to Dying Well.
The tool can be downloaded and used locally, and includes a range of supporting information and links to national policy and guidance. Additionally, it provides an easy-to-use dashboard and improvement plan to aid strategy and service development.
