The National Audit of Dementia's planned activities for Community-based Memory Assessment Services in 2026.
The following activities are planned for Community-based Memory Assessment Services in 2026. The timeline is subject to change based on the availability and quality of routine Mental Health Services Data Set (MHSDS) data (England), Performance data (Wales), and Health and Care Jersey data (Jersey).

All community-based Memory Assessment Services are eligible to participate. This audit is included in the quality account for Mental Health Trusts.
Please complete one form per team/service. Please note that this form is not for neurology-led clinics or other acute hospital services.
A Healthcare Quality Improvement Plan was developed for Dementia Diagnostic Services in consultation with NAD's Steering Group, Implementation Group, Lived Experience Group and Clinical Audit Leads.
Additionally, the NAD team consulted with a volunteer panel of Audit Leads to gain insights on data flows and recording.
The draft plan outlines the goals and key metrics prioritised for audit and is subject to change/further development related to feasibility.

Building on from NAD's 2025 Quality Improvement (QI) webinar series, the team will support QI groups with their planned activities throughout 2026. Each group will be led and facilitated by volunteer Audit Leads.
NAD's next QI webinar for memory assessment services will take place on Tuesday 2 June, from 12:00 - 13:00. This session will be led by Spencer Dinnâge and Nancy Smith from Essex Partnership University NHS Foundation Trust. They will sharing learning from the use of the Frailty, End of Life and Dementia Assessment (FrEDA) template, which supports coordinated care from referral through to end of life, led by a named Community Dementia Nurse Specialist.
