ACOMHS Questionnaire Entry
College Accreditation and Review System (CARS)
All ACOMHS member services will be provided with unique survey links in order to complete ACOMHS questionnaires online. Questionnaires for service users, relatives, friends and carers, and referrers are also available on paper.
ACOMHS uses the following questionnaires as part of the self-review data collection:
- Service User Questionnaire
- Relatives, Friends and Carers Questionnaire
- Staff Questionnaire
- Case Note Audit
- Referrer Questionnaire
Your unique survey links will have been sent to the lead service contact, but if you have lost these you can resend them to the lead contact by logging in to CARS and going to Review Visit > Survey Dashboard > Resend Links
ACOMHS Member services can log in to CARS at any time during their self review period to complete their self review workbook (rating themselves against each of the standards), monitor survey returns and download their report.
Feedback from people who have used the service, and their relatives, friends or carers
If you have been asked to complete a questionnaire by your local community mental health service, because you have used the service or you are a relative, friend or carer of someone who has, you can fill in the questionnaire online by using the link or the paper copy provided.
If you do not have a link, you can contact your local community mental health team or the ACOMHS team to request one.
Alternatively, you can contact your community mental health team or the ACOMHS team to request a paper copy of the questionnaire.
The questionnaire takes 5-10 minutes to complete.
Feedback from people who refer into the service
If you have been asked to complete a questionnaire by your local community mental health service because you have referred people into this service, you can fill in the questionnaire online by using the link or the paper copy provided.
If you do not have a link, you can contact your local community mental health team or the ACOMHS team to request one.
Alternatively, you can contact either your community mental health team or the ACOMHS team to request a paper copy of the questionnaire.
The questionnaire takes 5 minutes or less to complete.
